# Feedback Form

> Collect customer ratings and comments from the menu.

Use Feedback Form settings when you want customers to send feedback from the menu.

## Turn It On

Enable the feedback form only when someone is ready to review submissions.

## What To Ask

Keep feedback simple:

* Rating.
* Comment.
* Optional contact details if follow-up is needed.

Short forms usually get better responses.
